Payroll Deduction...
Thank you for your interest in setting up a payroll deduction pledge! The process for payroll deduction has changed from past years,
and you now must enter your own information. Listed below are step-by-step instructions you may reference to assist you in filling out
the form.

Tips for filling out the IUF Payroll Deduction Form...
Section 1: Logged In As |
- Your university ID, name and email address should automatically populate.

Section 2: Donor Information |
- Gift Recognition Name(s): List anyone that you would like to be recognized by this pledge,
such as yourself or you and your spouse/partner.
- Department: Enter Business
- Campus: Choose Bloomington
- Work Address: Enter your work address. Your work phone number should automatically populate.
- Home Address: Enter your home contact information, including street address, city, state, zip, and phone number.

Section 3: Employee Pay Type |
- Deduction is:
- Additional – Choose this option if you are adding an additional pledge to any current existing pledge(s).
- New – Choose this option if you have no existing pledge(s) and are creating a pledge for the first time.
- Replace all current – Choose this option if you would like to cancel any remaining current pledge(s) that you may have, and replace with the pledge you are entering.
***Please note: Your pledge will begin on the next payroll period. If you are completing an existing pledge,
and you enter your new pledge as Additional, there may be overlap in your pledges. To avoid overlap, do not enter your new pledge
until your existing pledge has ended, or choose Replace all current. If you would like information about your current pledge,

- Pay Type:
- 10 month
- 12 month
- Bi-weekly
- Number of Deduductions:
- Indefinitely – Choose this option if you would like your pledge to continue indefinitely or until you provide written notification to the IU Foundation to end your pledge or are no longer employed at Indiana University.
- Specific number – Choose this option for a specific number of payroll deductions. You must choose a minimum of 4 payroll periods.

Section 4: Deduction Information |
- Campaign Code: Enter BUA 0374-10
- To set up your pledge:
- Unit: Choose Kelley School of Business (B011). The options are listed alphabetically by code.
- Account: Choose what KSB account you would like to designate your pledge to. These options are listed alphabetically by name. Some commonly used accounts are:
- Kelley School Shareholders Fund (I38B011062)
- Accounting Departmental Fund (I32B011297)
- Business MBA Program (I32B011669)
- Business – Graduate Career Services (I32B011AP2)
- Business – Marketing Fund (I32B011370)
- Business – UG Career Services (I32B011628)

- Amount per pay period: Enter the amount you would like to be deducted from your paycheck per pay period... when finished, click Add.
- If you would like to enter a second pledge to another designation, repeat the above process.

Section 5: Review/Signature |
- Check the box to electronically sign the form.
- Click Submit.
